Rating scales

This page includes a number of links to various symptom rating and screening tools, and information about them. They are here primarily so for clinicians to down load and use with their patients. They are, however, freely available for anybody to use though please note that NCMD is not able to provide clinical input for individuals who complete one of the tools or have concerns about their mental health. In such circumstances you should speak with your local mental health team, if you have one, or your GP.
